Filing the initial application - The decision to file an application for disability benefits is always difficult. Aside from the many tasks involved and forms to complete, there is the emotional aspect of acknowledging that you need help.

A Social Security or SSI application is most easily initiated by calling the Social Security Administration (800) 772-1213 and making an interview appointment. You can also locate your local social security office at http://www.ssa.gov/regions/regional.html. Anyone who is unable to get to a local Social Security office for an appointment can request a telephone interview appointment.

When you go to your appointment, it is most helpful to have the following information:

* Your doctors' names, addresses, and phone numbers.
* A list of all prescription medications, with dosages.
* a list of all hospitals, clinics or institutions, addresses, and dates treated.
* List of all employment for the past 15 years.
* You will need birth certificates for yourself, spouse and children.
* Your Social Security number.

If you are applying for Supplemental Security Income benefits you also need the following:
* Information about your mortgage or lease.
* Payroll slips, bank books, and other information about assets.
